To follow is a four-section online form containing all of the mandatory information needed to report a First Report of Injury (FROI). You will be asked to complete each of these four sections completely, or you will be prompted for any missing information. As always, you can FAX completed BWC FROI forms to CareWorks, toll-free, at 1-888-711-9284. Or, you can report FROIs to CareWorks over the phone by calling our Customer Service Department, toll-free, at 1-888-627-7586.
