“As a field nurse case manager, I bridge communication gaps by attending appointments to eliminate care delays. Helping patients navigate and find relief during these highly stressful events is truly the best part of my job.”
Challenge
63-year-old custodian, *Jeremiah, sustained a non-displaced vertical fracture of the right patella that required prompt orthopedic evaluation and specialized treatment. Language barriers presented challenges in communicating the treatment plan and coordinating care, while delays in obtaining an orthopedic consultation could have prolonged recovery and postponed the initiation of appropriate treatment.
Recognizing the need for expedited specialty care and ongoing support, Jeannette was assigned to bridge communication gaps, coordinate services, and ensure the injured employee received timely and appropriate medical treatment.
Solution
Jeannette immediately took action to secure an urgent orthopedic consultation. Leveraging an established professional relationship Jeannette had with *Michelle, a team lead and coordinator at the orthopedic and spine provider, Jeannette successfully expedited the specialist appointment, minimizing delays in care and ensuring Jeremiah was evaluated promptly.
During the orthopedic evaluation, the specialist confirmed the diagnosis of a non-displaced vertical right patella fracture. This timely assessment provided diagnostic clarity and eliminated the need for redundant testing, allowing treatment efforts to remain focused on recovery rather than additional diagnostic procedures.
To support healing and protect the injured knee, the specialist fitted Jeremiah with a knee immobilizer and recommended durable medical equipment. Jeannette immediately coordinated the durable medical equipment request and continued to actively monitor the authorization and delivery process to ensure Jeremiah received the necessary equipment without delay.
Result
Through proactive intervention and strategic care coordination, Jeannette secured timely orthopedic treatment, accelerated diagnostic confirmation, and helped avoid unnecessary medical expenses associated with duplicate testing. Because of this, Jeremiah was able to receive appropriate immobilization and a clear treatment plan focused on fracture healing and functional recovery.
