Case manager assists injured worker far from home

Challenge

A student employee was seriously injured in a serious multi vehicle accident resulting in multiple fractures. He was rushed to the ER where he underwent surgery. The injured worker planned to return to his parents’ home over 350 miles away to recover.

Solutions

A telephonic case manager, Becky, was assigned to coordinate his medical care. Becky reached out to the family and answered the family’s questions and discussed their concerns. She worked with the injured worker’s family and all providers to coordinate his transfer to an in-patient rehab and then to his parents’ home.

She also coordinated in-home physical therapy once he returned home so he could continue to progress. Two months after the accident, he transitioned back to West Texas and Becky once again helped by coordinating continued physical therapy at a clinic close to his campus. She worked with the provider to secure modified duty work releases so the injured worker could return to work when the school semester started.

Results

Just three months after the accident, he returned to full duty with no restrictions and was released from care. Throughout his recovery, Becky not only collaborated closely with the family and providers, but also kept the adjuster and employer updated on the treatment plan and work status.
