“From the beginning of my nursing career caring for cardiac patients, I knew that case management was the path I wanted to pursue. I found my niche in occupational medicine, where I can blend my passion for patient care with an understanding of business operations. This role allows me to leverage years of experience and knowledge to provide efficient, compassionate, and comprehensive healthcare management. Helping people achieve the best possible outcomes while supporting employers and healthcare teams makes this work both meaningful and rewarding.”
Challenge
In January 2026, a 40‑year‑old tire store manager was rear‑ended at a stoplight by a vehicle traveling at least 50 mph. Despite the force of the collision, the emergency room performed only a basic work‑up and discharged him with “normal” spine x‑rays and instructions to expect soreness.
He was medically cleared and therefore returned to work the next day. However, his wife immediately noticed alarming changes—memory issues, word‑finding difficulty, and profound fatigue. The store assistant also observed concerning behavior and reported it to the employer’s headquarters, prompting a referral to an occupational clinic 6 days after the initial accident.
On January 19th, he was seen at an occupational clinic, where he again felt the provider minimized his symptoms, diagnosing him with whiplash only. His wife, still deeply concerned, drove him straight to another emergency room in the area. This time, a full trauma work‑up revealed the true severity of his injuries: T1–T4 vertebral fractures and a head injury with loss of consciousness, confirmed by in‑cab camera footage.
After reviewing records upon assignment and speaking to the claimant, Roxanna immediately realized her challenge was twofold:
- Help the claims adjuster and employer understand that this was a catastrophic injury, not a minor one.
- Rebuild trust with the claimant and his wife, who felt dismissed, minimized, and unheard by the healthcare system.
Solution
From the moment Roxanna was assigned to this case, she knew proactive intervention was essential to the success and recovery of this claimant.
Roxanna immediately secured an occupational medicine provider who is known for his thorough approach, comprehensive occupational evaluations, and strong community connections for specialty referrals. Given the complexity of the claimant’s injuries, Roxanna quickly arranged consultations with support of the claims adjuster and employer.
Roxanna attended appointments, facilitated communication, and ensured every provider had accurate, up‑to‑date information. She tracked every specialist referral and stayed ahead of the rapid flow of diagnostics and treatment requests. Her role became the central point of coordination, ensuring that the employer, claims adjuster, and medical team all understood the full scope of the injury and the need for timely, specialized care.
She also spent significant time building rapport with the store manager and his wife, validating their concerns, and helping them feel supported after their earlier experiences of being brushed aside.
Result
Through Roxanna’s persistent advocacy and coordinated oversight, the store manager has now received appropriate treatment for all accepted body parts and continues with his medical recovery. Due to quick case management intervention, this claimant is healing properly without significant pain in his mid-back. She has helped him to understand that although there are some permanent components to his injury, this is not a roadblock to returning to his job. In early June, the claimant returned to work full duty, something that just a few months ago felt like an impossible goal.
What began as a “simple” referral evolved into a complex, high‑level case requiring rapid escalation, multidisciplinary coordination, and strong relationship‑building. By recognizing the true severity early and advocating relentlessly, Roxanna helped transform a potentially prolonged and fragmented recovery into a structured, supported, and successful rehabilitation journey. This case demonstrates the powerful impact of proactive field case management—and the difference early advocacy can make.
