Transforming Recovery Through Early, Coordinated Case Management

circles

Meet Summer Woods, RN, CCM

With over 20 years of workers’ compensation managed care experience, Summer Woods embodies the power of choice and is a trusted choice for complex and catastrophic cases. Her strong clinical judgment, strategic resource planning, and multidisciplinary approach have made her a highly requested field case manager and a valued partner to claims examiners.

Before joining Careworks, Summer managed catastrophic claims nationwide for a managed care TPA, coordinating comprehensive medical and support services. Her leadership abilities led to advancement into management, and in 2018 she was honored as one of the organization’s top three clinicians with the Commitment to Excellence – Most Inspiring Nursing Award. She was also selected to work directly with the CEO on a new healthcare initiative.

“Case management is about guiding people through complex situations with clarity, compassion, and purpose. I take pride in making sure every detail is handled efficiently so that my clients feel supported and are consistently positioned to achieve optimal outcomes.”

Challenge

In complex injury management, early intervention and coordinated oversight can dramatically influence recovery. A recent catastrophic case managed by Summer, an experienced field case manager, demonstrates how proactive involvement can completely reshape an injured employee’s trajectory.

In October 2025, a 75‑year‑old stock associate fell from an eight‑foot ladder while retrieving a Christmas tree in the stockroom. The accident resulted in multiple traumatic injuries, including an open left radial fracture, facial and pelvic fractures, vision changes, and an intracranial hemorrhage. He required emergency surgery the following day.

The severity of his condition and involvement of numerous specialties—orthopedics, neurology, ENT, ophthalmology, neuro‑ophthalmology, hand surgery, and rehabilitation—created significant risk for fragmented care, delays, and a potentially multi‑year recovery.

Solution

Within a day of being assigned the case, Summer was already at the hospital for the initial meeting. After learning that the claimant had just returned from surgery, she immediately initiated early intervention. Summer met with the claimant, his family, and the hospital staff to establish a unified care plan and educate them on Sedgwick’s case management process.

Summer coordinated his transfer to an inpatient rehabilitation facility and arranged referrals to seven outpatient specialists, attending appointments and ensuring all treatment followed evidence‑based guidelines. Recognizing safety concerns related to his brain injury, she arranged transportation and attendant care to prevent reinjury.

Throughout inpatient rehabilitation, Summer participated in interdisciplinary case conferences with therapy teams, occupational medicine, and facility case management to ensure treatment remained appropriate and medically necessary. Her continuous oversight supported timely transitions, including the claimant’s move to a brain injury day program in December 2025, and discharge home by January 2026. She maintained coordination of outpatient care to sustain recovery momentum.

Result

Summer’s strategic and highly coordinated approach produced exceptional outcomes. The claimant reached maximum medical improvement (MMI) with all providers except neurology, within three months post‑injury. Neurology MMI is anticipated at the April 15, 2026, visit—only six months after the accident, far surpassing the originally projected multi‑year timeline.

Her swift coordination of therapies and diagnostic studies through preferred vendors ensured timely access to care. Strong communication supported full claimant compliance, reducing overall file length. Summer also ensured treatment plans were promptly updated for all stakeholders and leveraged her relationships with specialists to secure rapid initial evaluations, preventing delays.

The employer described her oversight as next level,” underscoring the powerful impact of skilled, proactive case management.

Conclusion

Through her leadership and dedication, Summer transformed a complex, high‑risk injury into a cohesive and highly successful recovery journey—showing how coordinated case management can truly change lives.
